Dr. Chernyk's practice is committed to maintaining your confidence and trust, and maintains the following privacy policy to protect personal information you provide online and in your treatment sessions.

Privacy Online.  If you are going to submit information to us via the internet, there are a few things that you should know first. Although we do collect general information about how many people visit this site, we do not store cookies, collect, save or use any information that might identify you personally. If we do communicate with you via email, please be aware that email information may not be entirely secure. Emails and information contained in emails is kept confidential through password protection and by deletion of such emails when the site visitor’s request has been satisfied. We subscribe to the HONcode principles relating to health care on the net and are in compliance with the security encryption requirements mandated by HIPAA. A copy of Dr. Chernyk's HIPAA policy is available by request.  Personalinformation, such as your name, postal and e-mail address or telephone number, is private and confidential. The personal information you provide is stored in a secure location, is accessible only by Dr. Chernyk and is used only for the purposes for which you provide the information, such as to reply to your request or to send you requested materials.  Dr. Chernyk does not maintain a mailing list.  Personal information will not be released to third parties except to further the purpose for which you provide the information or if such release is required by law.

This site does not track, collect or distribute personal information about visitors.  It does compile aggregate statistics that show the number of visitors to the site, the requests we receive for specific information and the internet service providers that these requests come from.  These statistics are used to help us improve this website.  There is no "guest book".

Privacy in Treatment.  Generally speaking, the information which is discussed in your treatment sessions is confidential. There are however, several instances in which information about potential harm must be reported to the authorities. The circumstances under which we may be required to breach your privacy in treatment include the following:

  • If you indicate that you intend to seriously harm yourself or someone else
  • If a reasonable suspicion of child abuse or molestation exists
  • If you make your psychological status an issue in a court of law
  • Your clinician needs to consult with another professional about your treatment (in this case, your name and other identifying information will not be revealed);
  • If you are a minor, we are legally obligated to keep your parent or guardian informed of your progress. Both residential and non-residential parents, unless prohibited by court order, have rights to the minor's treatment information. This information can be released to the non-residential parent unless you provide Dr. Chernyk with written documentation that such access is to be legally restricted.
